To submit a report using the Internet, access the dedicated website using a PC or mobile device then enter your ID and password. Follow the instructions on the Create a New Report screen to enter and submit the details of your report.
To submit a report via telephone, dial the designated toll-free number (can be dialed from cellular and PHS devices as well). You will be asked for your company name, after which a D-Quest employee will listen to the details of your report.
To submit a report through postal mail, enter the details on the designated form then mail it to D-Quest.
If you submit a report via telephone or postal mail and wish to remain anonymous, D-Quest will hide your name and other identifying information and provide only the details of your report to your company.

For information about what items are eligible to be reported, please refer to your company's regulations and manuals. If you are still unsure, contact your company's general affairs or compliance department as appropriate. Since the defamation of another person's character is an ethical violation, please remember to always be respectful with your report details.

We use advanced security measures, including proprietary password technology and encrypted communication protocols, to ensure that data remains confidential and that reporters cannot be identified when submitting reports via the Internet. And since our system is designed to not normally store access logs (records of the IP addresses and other details of computers that access our servers), it is impossible for the identity of reporters to be deduced from a technical point of view as well. If you submit a report via telephone or postal mail and wish to remain anonymous, we won't ask for your name or other identifying information.

If I submit a report over the Internet using my employer's computer, will they be able to identify me?

As mentioned above, our helpline system is designed to prevent the identification of individuals who submit reports over the Internet. However, if the computer or network connection you use to submit the report contains special software that monitors its usage, there is a risk that you may be identified - not from our helpline system, but from that equipment on your end. If you aren't very familiar with your company's system-usage policies and security measures, we therefore recommend that you access the helpline via your own private device (such as a mobile phone) and Internet connection rather than that of your company.

Yes, you can give your real name whether you access the helpline via postal mail, telephone, or the Web.
If using the Web, you may choose to enter details such as your name, job position, and contact method in the section titled [If reporting using your real name].
You can even submit a report anonymously at first then decide to provide your name or other information during the process of exchanging information with the recipient (a supervisor at your company).
